Virtual Telehealth Appointment Instructions

To ensure a smooth experience for your virtual appointment please follow these steps:

1. Email Invitation: After scheduling your appointment, check your email for an invitation to log into our medical charting system, CharmEHR.

2. Complete Forms: Log into the emailed link to fill out the necessary consent form and provide your medical history.

3. Zoom Meeting Link: After submitting your forms, you will receive your Zoom meeting link for the consultation.

4. Prepare for the Meeting: Make sure your Zoom account is up-to-date. Please log in at least 10 minutes before your scheduled appointment.

Important Information:

State Restrictions for weight loss: Our telehealth weight loss services are available only to patients in Utah, Wyoming, Colorado, Montana, and Idaho.

Deposit: A $25 deposit is required to secure your appointment.
